Transitional Care

Transitional Care Project Team

The Transitional Care (TC) Special Projects team develops and promotes healthcare literacy for youth, adolescents, and college students to transition from pediatric care to adult care. The team works with physicians, residents, and medical students of UCLA pediatrics to create and maintain a clinical healthcare system for adolescents to begin managing their own health. In addition, the team is excited to continue its efforts in reaching the UCLA community to help students better understand their own medical care. As a smaller team, TC highly values collaboration. Members should be motivated to help young adults navigate the complex world of healthcare, and will also have the opportunity to pursue individual, creative projects if time allows. If you are passionate or want to learn about the landscape of patient care and health education, TC welcomes you!

TC Dental Resources Referral Packet

The doctors and residents at the clinic noticed that many of their patients were looking for dental clinics that offered treatments for patients with special needs and specific fears/anxieties. The Transitional Care Team worked on creating a digital dental resources referral list with all necessary information & details like location, contact info, cost, treatment description, customer review, etc. so that the clinic staff can easily pull out the list to refer their patients.
